Underarm Perspiration/Auxillary Hyperhidrosis
Hyperhidrosis is a medical disorder characterized by excessive sweating, which is a common chronic condition that almost 3% of the population faces. Hyperhidrosis usually affects the underarm (axillary) sweat glands, but it may also affect the palms of hands or feet, or the whole body in general. This is not just an ordinary sweat problem--hydrohidrosis is characterized by sweating excessively at all times in varying external conditions. Sweat is the mechanism used by the body to regulate temperature. However, hyperhidrosis is defined when sweating significantly exceeds the body's normal requirement for cooling.
During young adulthood or adolescence is typically when people begin to develop this condition. The cause of the disorder is unclear, however, genetics may be responsible. Studies have shown that one-third to one-half of patients have a family history of hyperhidrosis. Heavy sweating may increase due to stress, emotion, anxiety, tension, certain types of food, or exercise, but often occurs for no particular reason.
TREATMENT
Treatment for hyperhidrosis is dependent on the severity of the affected area and may require more than one method. Primary treatment methods for this disorder such as powders, ointments and electrical stimulation, may only offer temporary results that lasts just a few hours or sometimes up to a week. Surgical procedures such as endoscopic thoracic sympathectomy (ETS surgery) might be used to treat hyperhidrosis. Unforunately side effects of this procedure might include compensatory sweating on the thighs, abdomen, back and legs. Decreased heart rate and dry skin (particularly on the scalp, face and neck) are other side effects.
BOTOX® Cosmetic (botulinum toxin type A)
injection is the preferred treatment method. It is a minimally invasive procedure to help treat patients with severe primary axillary hyperhidrosis that was insufficiently treated with topical agents. Results may be seen within the first week after treatment. Studies have shown BOTOX® Cosmetic to be up to 90% effective and results can last up to 9 to 12 months before additional injections are required.
SmartLipo
is another treatment method that offers the longest lasting solution to hyperhidrosis, and is usually considered when other methods of treatments are not successful. SmartLipo is the most effective method to remove excessive sweating. Sweat glands are inactivated by light energy from the laser fiber, therefore patients do not need to come back for repeat treatments. This procedure is performed under local anesthesia enabling you to return to your normal everyday activities shortly after the procedure. SmartLipo has proven to be 70-90% effective for hyperhidrosis and results last up to 3-5 years.
Facial lines and wrinkles are a natural part of the aging process. As the production of collagen underneath your skin begins to decrease and the elastin fibers become weak this will cause the appearance of wrinkles. Although this is a natural part of the aging process, excessive sun exposure, frowning, smiling and squinting may also be a factor.
The two most commonly used methods for facial lines and wrinkles are injectable fillers and fat transfers.
While injectable fillers can decrease the appearance of wrinkles, they might not be suitable for everyone. There are possibilities of allergies to bovine collagen or you may develop an allergic reaction over time. Some injectable fillers can cost up to $800 per syringe and lasts less then one year, thus it is not very cost effective when compared to fat transfers. Regular injections can be considered a continuing expense.
Fat transfer is the perfect solution if you are looking to fill in your facial lines and wrinkles. A popular alternative to other wrinkle treatments fat transfers can plump up facial features or hands with a patient’s own fat. Fat transfers are a safe, natural, non-allergenic and lasting procedure intended to recontour your face or any other unwanted areas with wrinkles. Since you are using your own fat cells with fat transfers you can never have an allergic reaction because it is from your own body.
Longevity of fat transfers is usually seen in larger areas of non-movement, for example the cheeks. Fat transfers are also great for correcting atrophic aging of the hands. If you are looking for something to rejuvenate the hands and reduce the hollow and bony appearance then fat transfer is the perfect option. However, not every area of the body responds well to fat transfer. The breast is not a good area to do fat transfer due to the fact that it makes it difficult to detect breast cancer during mammograms.
Procedural Steps
The fat transfer procedure is performed in our office on an outpatient basis. Both the areas where the fat is removed and where the transfer is placed are put under local anesthesia. A small needle attached to a syringe is used to remove fat from the area with the most tightly packed fat, such as the thighs or abdomen.
When the fat is removed it is harvested, processed to remove any excess fluids, and then reinjected using another needle that is placed under your skin beneath the wrinkle. This process may be repeated until the desired appearance is achieved.
Harvesting fat cells for fat transfers and reinjection is performed in conjunction with liposuction. It is also possible for the surgeon to harvest a larger amount of fat to freeze and store for future injections if desired.
Benefits of Fat Transfer
Fat transfers can last up to 3 years. Fat transfer can be very cost effective when combined with another procedure. There’s no risk of an allergic reaction.
Expectations after Fat Transfer
Patients are up and about immediately following fat transfers. Any discomfort that does arise may be controlled with medication. There is typically little to no discomfort with fat transfers. If swelling does occur at the treatment site it may last up to 36 hours. There may be some bruising that can be easily covered up with make up. Over the first few months about 35-50% of the fat cells will be absorbed by your body. The remaining 50-65% will stay in place for years to come.
Your initial injection should last at least 6 months. To achieve results that will last for years, patients usually receive 3-4 treatments over a 6 month period.

How does the Smartlipo procedure work?
We use the newest 18W SmartLipo machine, which is highly safe and effective. The higher wattage allows for faster treatment and greater patient results. The energy released from the laser allows for tissue coagulation, thus inducing collagen retraction and tissue tightening.
A very small cannula (tube) approximately 1.0 mm to 2.0 mm in diameter containing a laser fiber is inserted into the skin. The cannula is moved back and forth delivering the laser's energy directly to the fat cells, causing them to rupture and easily drain away. The laser energy also interacts with the dermis, resulting in collagen stimulation. Due to the cannula's small size, Smartlipo is a minimally invasive procedure and can be performed under local anesthesia. The laser causes small blood vessels to coagulate immediately upon contact, resulting in less bleeding, swelling and bruising-and a quicker recovery time than conventional liposuction. Who is a good candidate for the Smartlipo Laserlipolysis procedure?
If you have areas of fat that will not respond to diet and exercise, then you're a good candidate for Laserlipolysis. Ideal candidates are in good health, of normal body weight and have realistic expectations of the results.
What body regions can be treated with Smartlipo?
Smartlipo can be a great adjunct to traditional liposuction procedures, although it does not replace conventional liposuction. It is ideal for treating small areas of the face, neck, arms, breasts (gynecomastia), and areas of loose and flabby skin (floppy skin).
How many treatments will I need?
For most patients, typically only one treatment is required. The session may last from 45 minutes to one hour for each area treated.
Do I have to do anything post-treatment?
Rest is recommended after the procedure and activity may be resumed gradually. Refrain from hot tubs and strenuous exercise for 2 weeks. The incisions will close up by themselves within a few days. The patient is placed in a compression garment the day after the procedure. Compression garments are an important part of the recovery process. The garment keeps the tissue together, provides comfort and support. Compression garments minimize swelling by helping to prevent fluid buildup. The garment should be worn for 24 hours a day for the first two days, and for 12 hours a day after that for 2-6 weeks. There may be significant bruising and some swelling that may last a few weeks. Final results may take up to three months.
When can I expect to see results?
Results can be seen as soon as a week with continued improvement over several months.
Does laserlipolysis provide lasting results?
Yes! Adults do not create new fat cells. When a person gains weight, the fat cells expand. The Smartlipo procedure permanently removes the fat cells. Should you gain weight, most fat will be deposited in areas where the procedure was not performed.
